This monster is balanced to fit in a high level of balance game. I have taken the liberty of not using the standard D&D rules for determining various statistics for this monster. As a result, it may seem a bit stronger (or weaker) than some SRD monsters of the same CR and that is fine. I think it will pose an appropriate challenge for characters of its CR.

Greasy Coating (Ex) Bacon elementals are covered (and filled with) delicious, artery-clogging bacon grease. Whenever the elemental is struck by an attack or fails a save against a spell, the attacker or caster immediately has a Incendiary SlimeCM spell cast on their hands. This will likely cause them to drop their weapons or have difficulty grabbing spell components. The save DC is Constitution-based.
Pork-Powered Durability (Ex) Bacon elementals are powered by the elemental powers of pigs, boars, and even dire boars. Sometimes turkey bacon makes its way into their bacony mass, but they would never admit it. The power of pork grants them + 20 hitpoints.
Grease Slide (Ex) The bacon elemental slides around leaving a trail of grease. It ignores all difficult terrain, gets a +4 dodge bonus to AC when moving, and leaves a patch of grease like the Incendiary SlimeCM spell for each square it leaves (caster level equal to hit dice). When moving downhill, its speed increases to 50 ft. (10 squares). The save DC is Constitution-based.
Delicious Smell (Su, mind affecting) Bacon elementals smell amazing. Any creature within 30 ft. (or 60 ft. if they possess the scent special ability) must make a Will save DC 15 or else be overcome by the urge to take a bite out of the bacon elemental. They must spend their next turn moving in the most efficient way they can towards the bacon elemental and making a bite attack against it if they have one. If they do not, treat it as an unarmed strike instead (with their mouth).

Any creature that eats the bacony deliciousness of a bacon elemental must make a Fortitude save DC 15 or contract coronary heart disease.

Disease (Ex) Coronary heart disease, Fortitude DC 15, incubation period 1d3 days, damage 1d3 Con. The save DC is Constitution-based. Any creature exposed to the disease when already infected must make a Fortitude save or suffer the effects of the disease immediately.

Ahead of you is what looks like something both repulsive and yet somehow delicious. A massive pile of sizzling bacon slides across the ground towards you taking the form of a humanoid. The smell that makes breakfast so wonderful fills your nose and you can't resist the urge to help yourself...

A bacon elemental is a pile of bacon that has become animated with an elemental spirit. Often this happens when specialty chefs power their stoves with a fire elemental and make the fatal decision to fry up a couple hundred pounds of bacon. Some food scholars believe this is utter hogwash and that the true origin of the bacon elemental comes from an elemental plane of bacon.

Whatever their origin, they are very frustrating foes to face. They slide around the battlefield spreading flammable grease everywhere. Any opponent that attacks them is at risk of getting greasy fingers. All around, things become dangerously comical when a bacon elemental enters the fray.
