You transform a disease into a spirit of malice and wrath. First, you transform the disease itself into a spirit; treat this as a SRD:Remove Disease effect, except that this is a transmutation effect with a range of medium, is not considered harmless if the target is unwilling, and may instead without a save purify all terrain and unattended objects in a 5-ft square.
If you successfully remove the disease, you transform it into a plague spirit. The spirit spitefully hurls itself forward, dealing 1d6 points of untyped damage to every creature in a line with a distance of your close range. The line originates from the target purified, not affecting them. Protection against possession protects normally against this effect, and a successful fortitude save negates the damage entirely. Any create taking damage from this effect is considered to have been exposed to the disease as per the normal rules for that disease. The short-lived spirit perishes at the end of this effect.
Material Component: An example of the plague in question, either a person infected by it, or an object of terrain containing it.
