A rubbery purple worm with a three lobed grabbing tail and a three lobed head that hides a glassy cloudy eye which shimmers with psionic energy, and smaller long eyespots outside of its head. Staring at it makes your head hurt.

Strange creatures some suspect were born of illithid experimentation or discarded beholder eyestalks, these creatures are actually mostly harmless. They are psychic scavengers, seeking out intelligent minds and slowly absorbing their ambient mental waves. Direct feeding has harmful effects on the mind, dealing psychic damage and provoking a strong feeling of paranoia and vertigo. They lurk in the underground, poking out of cracks in the wall to feed on unsuspecting victims.

It is about a foot long and weighs 3 lbs. It understands Common and Aklo (or another language associated with aberrations).

Combat

Ill suited for combat, a wormgaze will only fight if it cannot run. It can only rely on its psi-like abilities and its psychogaze to keep opponents at bay.

Amphibious (Ex): A wormgaze can breathe both air and water.

Grab On (Ex): A wormgaze is usually poor at grappling, but its tail has velcro-like grips that let it hang on surfaces and creatures. When making a grapple attempt it can try to grab on, ignoring its size penalties, using its Dexterity in place of strength, and adding a +8 racial bonus. When grabbed on it is attached to a creature who is not considered grappled. You have soft cover from the creature you are grappling, and creatures attacking you with ranged weapon has a 50% chance of hitting the grabbed creature instead. You may use Grab On to defend against attempts to break the grapple.

Psi-Like Abilities (Ps): At will-detect psionics, my light; 2/day-distract (DC 14). Manifester level 2nd. The save DC is Wisdom-based.

Psychogaze (Su): As a standard action with Concentration, the wormgaze can stare at a creature within 60 ft and the creature must make a Will save or be At Bay from the wormgaze as long as it concentration. A successful save renders them immune for 1 minute. Regardless of the save or immunity, the creature takes 1 point of non-lethal damage. This is a mind-affecting effect. This is a gaze attack. DC 14. The save is Wisdom based.

See Ethereal and Invisibility (Su): The wormgaze's central eye can see invisible and ethereal creatures, as per see invisibility. Ethereal creatures are hazy with concealment, but the wormgaze has no means of directly interacting with them.

Tactile Link (Su): The wormgaze has telepathy out to a range of touch. If physically touching a creature it can communicate. Most seem craven and nervous, and have few things to talk about beyond the search for interesting minds.

Familiar

Wormgazers can be used as familiars for spellcasters with the Improved Familiar feat and at least caster level 5th. When used as familiars they usually Grab On, mounted upon the shoulder, and act as a wayfinder against invisible or ethereal creatures.
