This spell transforms the target's minor physical qualities (such as hair, skin, eyes, voice and features) within the normal range of his race at the caster's choosing. The target's equipment changes size to fit any necessary alterations, but does not otherwise change in appearance.

The target may dismiss the effects of ''disguise form'' as a [[SRD:Standard Actions|standard action]], leaving him [[SRD:Dazed|dazed]] until the start of his next turn. If the spell is not dismissed or dispelled by the end of the duration, the effects are then applied [[SRD:Duration (Spell Descriptor)#Permanent|permanently]].

If the target uses this spell to create a thorough disguise, he gains a +10 circumstance bonus on his [[SRD:Disguise Skill|Disguise]] check.
