The target is cursed with a swollen tongue which jumps in the way of the creature's teeth.  Whenever they attempt to speak they take 1d6 points of damage per attempt, including with vocal components which may disrupt spellcasting with a -10 penalty on the Concentration check to maintain the spell due to the sudden pain.  This damage bypasses DR.  It is an effective means of de-facto silencing a creature lest they risk damaging themselves.  On a successful save it only lasts 1 round.
