The subjects immediately become helpless in horrible agony unless they succeed on a Fort save. They remain this way for 1d4 rounds, and only [[SRD:Heal|''heal'']], [[SRD:Limited Wish|''limited wish'']], [[SRD:Miracle|''miracle'']] or [[SRD:Wish|''wish'']] can end this process otherwise. If not ended prematurely, at the end of the duration, the subjects are killed, with no saving throw possible.

If the Fort save is passed, creatures still become helpless for 1d4 rounds, but do not risk death. Additionally, during any round in which these creatures are helpless, they are dealt 2d6 damage. This can be removed by any spell or ability that restores hit points or removes a condition that is at least 3rd level.
