Caster's Headache
Infection: Inhaled1

  1. Just being within 5 ft. of a creature who is sick with Caster's Headache for more than a minute exposes you to the disease, unless extreme wind conditions favor you.
  2. You gain 1 permanent spellstrained level each day you fail your Fortitude save, which are removed together with the disease.


Sometimes, when a spellcaster is Disintegrated, it leaves behind a green cloud of magic gas that spreads Caster's Headache; the most common start of a Caster's Headache outbreak is after carriers who are sick transmit it to others by just being near them.

Better Disease Implementation (3.5e Variant Rule)[edit]

Caster's Headache Inhaled, 1 day cycle
Your magic has begun to devour itself; you feel the magic within slowly draining away.
You are cured. If you cast more than 1 spell per 2 rounds, you gain 1 permanent spellstrained level. You gain 1 permanent spellstrained level per disease stage. Up to x stages, each with no new effects past stage 2 (to a max of x = caster level of victim). (Stage x+1) You disintegrate into a green cloud of gas, exposing all within 100ft to the disease.

Cycle: 1 day

Stage 1:

You feel weakened whenever you use magic.

Whenever you cast a spell without a full round with no spellcasting before, you gain 1 permanent spellstrained level.3

Stage 2:

Each day, your inner magic burns a little more.

You gain 1 permanent spellstrained level4 per disease stage.

Stage 3+:

Your magic is about to implode your body and soul.

Up to x stages5, each with no new effects past stage 2.

Stage x+1:

With a last scream, you leave this world a last gift in form of a deadly cloud.

You disintegrate from within and die. Your remains turn into a green mist that spreads with a speed of 20 ft per turn from your place of death until it has filled all available space up to 100 ft away. Any creature with a caster level that passes through this death mist must make a Fortitude save or be infected itself (DC is equal to the victim's caster level + its Charisma modifier).

A moderate wind (11+ mph) disperses the fog in 6 rounds; a strong wind (21+ mph) disperses the fog in 2 rounds.

  1. Just being within 5 ft. of a creature who is sick with Caster's Headache for more than a minute exposes you to the disease, unless extreme wind conditions favor you.
  2. Any creature without a caster level is completely immune to this disease.
  3. These permanent spellstrained levels can not be removed by any means, but will disappear when cured of the disease.
  4. These permanent spellstrained levels will follow your disease stage; they grow and fall with the disease.
  5. Unlike other diseases, Caster's Headache does not have a fixed set of stages. Instead, it has up to x+1 stages, where x is equal to the caster level of the victim, and each stage has no new effect after stage 2 (other than increasing the effects of stage 2) until stage x+1.
