Not all creatures are born with manipulating hands. Some creatures are quadrapeds, all feet and no arms. Others have arm-like limbs, but they end in tentacles or other things which aren't hands. Due to their inability to hold items, they could not benefit from the various held magic items in the world. Fortunately one mad scientist knew how to fix that: thumbs for all.
The opposable thumb graft uses tissue samples from both the host creature and any thumb-bearing creature (usually a human), crossbreeding the genes magically. The graft is then grown onto the paw, tentacle, or whatever passes for a front pair of limbs. Regardless of how many thumb grafts are applied, only what passes for forelimbs can hold items. Placing thumbs on your feet will give you odd feet but won't let you wield more than two hands worth of items. They may hold items, swing weapons, and do anything else one expects with hands. They do not impose any penalty walking on the modified limbs.
Many creatures which get this graft also use their newfound "hands" as feet as well, such as most animals like the blink dog or similar. They cannot use their hands and move more than 5 feet in the round as they need all four limbs for effective movement. If the creature would be stable without those hands (such as with a centipede) then they may move and attack as normal.
If you were unable before, you now are able to wear gloves and rings. The price is for one thumb, though creatures usually get a pair.
