This is homebrew, you may be looking for the template in Savage Species.

Multiheaded[edit]

Because of magical mishap, strange genetics, hydra blood, or toxic mutation you have more heads than what is normal for your kin.

Creating a Multiheaded Creature[edit]

This is an inherited template which can apply to any creature which possesses at least one head. Rarely it may be acquired through strange means.

Attack[edit]

If the creature possesses a bite or gore natural weapon (or other head-based natural weapons) when they acquire this template, it has a bite or gore on each of their heads.

Special Attacks[edit]

Interactions with Other Special Attacks: If a multiheaded creature has a breath weapon, all heads gain a breath weapon and breath simultaneously when the breath weapon is used. Overlapping breath weapons overlap, they do not stack, damage. However, it may still be useful as they will be forced to make multiple saves against the breath weapon. You can also aim your breath weapon in multiple directions to cover a large area.

Multiaction (Ex): If you have at least two heads, you can perform a physical action and a single mental or vocal action in the same action. You can use psionics, or spells with the no somatic component with your extra action. Alternatively, you can get an extra attack at your highest attack bonus as part of the action instead. This stacks with haste. No matter how many heads you have you can't get more than one multiaction a round.

Special Qualities[edit]

Ambidexterity (Ex): Multiheaded creatures have no off-hand, and deal full strength on off-hand attacks and secondary natural attacks.

Many Eyes: Multiheaded creatures with at least two heads deny the +2 bonus on attack from flanking (but can still be flanked), and they gain +4 on Listen and Spot checks per additional head. If they have at least four heads, they cannot be flanked at all.

Abilities[edit]

+2 Wis on obtaining the template for the first time. If you have more than one head you do not get additional ability bonuses.

Feats[edit]

Multiheaded creatures gain Perfect Two-Weapon Fighting as a bonus feat. If you have more than three arms, you gain Perfect Multiweapon Fighting instead.

Challenge Rating[edit]

+2 for an extra head, and an additional +1 per additional head.

Level Adjustment[edit]

+2 for an extra head, plus an additional +1 per additional head.
