The claw marks on your shoulder begin to burn. It's a small injury, so why does it hurt so much?
This spell can only affect creatures which are already injured. You aggravate their wounds dealing 1d6 negative energy damage. This damage can be non-lethal if you desire, and it has no effect on creatures healed by negative energy. The spell cannot kill, leaving creatures at a minimum of 1 hp when dealing lethal damage. It is considered a pain effect, and immunity to pain confers immunity to this spell.
