The beautiful figure before you is marred by hands which are clearly far too large for their body.
Developed by an appearance obsessed sorcerer known as Yau Wei, it's uncertain if this was a botched spell, or a purposeful curse designed to mar the appearance of others. In any case, the spell found tactical use elsewhere and spread for combat purposes. A creature who fails their saving throw has their hands grow abnormally large. On one hand (no pun intended), it permits the creature to use items and weapons one size larger than it, and lets them perform combat maneuvers such as grappling as if they were one size larger. On the other hand, the creature takes a 1d4 penalty on all checks which employ the hands, including attack rolls, due to the clumsy nature of these engorged limbs.
The enlargement does stack with enlarge person as it is specific to the hands, but not with Powerful Build, Monkey Grip, and similar abilities. It does not augment the size of held items automatically as per enlarge person.
